The overhanging upper eyelid is a feature of appearance, predetermined by heredity, pathology or race. In medicine, this phenomenon is called ptosis and is often referred to as pathological conditions if it manifests itself on one side or leads to a decrease in visual acuity. After the age of 30-35, age-related drooping of the eyelids can develop, caused by aging and weakening of the tissues.
Pronounced blepharoptosis (more than 2 mm) is characterized by the development of a number of ophthalmic complications, including strabismus, conjunctival irritation, and lacrimation. The patient develops a habit of constantly tilting his head back or raising his eyebrows to maximize the opening of the palpebral fissure. This condition cannot be called life-threatening, but it causes significant discomfort.
Why does the upper eyelid droop?
At its core, the overhang of the upper eyelid is a polyetiological disease, that is, there can be many reasons for its development. The disease occurs against the background of a tension disorder of the circular muscle of the eye, the function of which is the rapid closing and lifting of the eyelids. Another provocateur of the formation of a skin fold can be a decrease in the tone of the autonomic nervous system.
A hereditary eye defect with an overhanging upper eyelid manifests itself from the first weeks of a baby’s birth. The gene responsible for the formation of this pathology has not yet been discovered. Blepharoptosis often affects one eye, but it can also be bilateral. You can detect it when observing a child – the baby often wrinkles his forehead, trying to raise his eyelids, throws his head back.
Genetic disorders that caused the impending upper eyelid:
- Asian eye shape;
- underdevelopment of the circular and raising upper eyelid muscles;
- an anomaly in the structure of the nerves of the skull responsible for eye movement.
For the impending upper eyelid with congenital defects, one can take dermatochalasis, drooping of the eyebrow, and a smaller than usual volume of the eyeball. Treatment of blepharoptosis should be carried out at an early age. This will help maintain vision and avoid complications.
There are much more acquired causes due to which an overhanging upper eyelid appears. In addition to problems with the oculomotor muscles and nerve, the following factors can provoke the omission of the skin fold:
- age-related changes;
- fatty hernia on the upper eyelid;
- weakening of skin tone;
- neoplasms in the eye area;
- scars that do not allow lifting the eyelid;
- eyebrow ptosis;
- traumatic injury.
The upper eyelid also hangs heavily after unsuccessful ophthalmic operations for cataracts, implantation of corneal segments, and elimination of glaucoma symptoms.
The risk group includes patients who have undergone botulinum therapy of the forehead area. Penetration of the toxin into the oculomotor muscles causes them to relax and lower the upper eyelid.
The main symptom of blepharoptosis is a well-defined overhang of the upper eyelid, covering part of the eye. It can be complete or partial and is accompanied by the following manifestations:
- irritation of the conjunctiva;
- fuzziness or splitting of the picture;
- weakness, lethargy of the upper eyelid;
- rapid eye fatigue.
A person with an overhanging upper eyelid is often tormented by headaches, lacrimation, and papillary conjunctivitis may develop. Patients have a shortening of the palpebral fissure, eversion of the eyelid during sleep, underdevelopment of the upper orbit, early wrinkles on the forehead.

What to do if the upper eyelid began to hang? You can try to do without surgical intervention, but on one condition – if blepharoptosis is not a congenital anomaly. If the skin fold hangs due to paralysis of the muscle that lifts the upper eyelid, surgical treatment is carried out according to the Hess method. During the operation, the eyelid is pulled up to the frontal muscle and secured with a suture. The wound is covered with a sterile bandage.
After the operation, the patient can go home the same day – upper eyelid surgery does not require a long stay in the hospital.
Recovery after blepharoplasty is quite fast and almost without complications. In addition to the expected consequences (edema, bruising), in rare cases, the patient may develop inflammation of the conjunctiva, lacrimation, and pain in the eyes. To avoid such complications after removal of overhanging skin over the upper eyelid, strict adherence to all doctor’s prescriptions will help.
What not to do during the rehabilitation period:
- wear contact lenses;
- touch and rub your eyes;
- read, watch TV, work at a computer, write;
- go to the bath, sauna or solarium;
- swim in a pool or open water;
- apply makeup.
The recovery period lasts 3-4 weeks, but the final result can be assessed only after 1.5-2 months, when the swelling completely subsides. Only then will the patient see that the skin no longer hangs over the upper eyelid, the look has opened and rejuvenated.
Prevention of overhanging of the upper eyelid
If we talk about acquired blepharoptosis, then the following recommendations will help to prevent, or rather, postpone its appearance:
- observance of the daily routine, a full eight-hour sleep;

- regular exercise for the paraorbital zone and self-massage.
Upper eyelids hanging over the eyes will appear much later if you follow facial expressions, make less grimace, maintain good health and mood, avoid stress and overwork. When the first signs of skin fold prolapse appear, you should take care of the tissues of the paraorbital region or contact a cosmetologist.
You can also get rid of the overhanging upper eyelid at home, if the condition is not genetically predetermined or painful. Otherwise, only blepharoplasty will help. The result of the procedure will last at least 5 years, but in most cases it takes 10-15 years before the next operation.
